Well, heart attacks happen when a player unsubs or asks the PD to kill a character, so if you don't do either of those things, it won't happen.

Implementing a ships/town bell. Just like when a character can hear someone begin breaking a lock, they can also hear a town bell or ships bell ring. Normally used for town meetings, religious services or if trouble is brewing. Or as a way to let sailors know that their watch onboard ship is ready to commence or is over with.
That way no one has to run around knocking on all the doors trying to notify people of what is going on. Warning!!! Bell is ringing! Better go see what's going on!

Inventions
Unique items can be invented, one for each skill apart form strength and fighting (project details supplied by RD). There will be one inventor for each of those items (a random character who is expert at the specific skill). Only the inventors know how to make the item they invented (can start the necessary project(s) and know the requirements). This is done by supplying the 'magic word' only he/she knows in the process of starting the project(s). Other characters will not even know the item can be made!
The inventor can choose to spread the word in game and let other chars create the item too (when they are experts!). Or not

Twenty in-game years after the item is first completed the knowledge of how to make the item will have spread anyway, so anyone can make the item (but must still be an expert at the skill). At that time the item can be added to the build menu and details can be shown in the wiki. After a longer time the expert requirement can be lifted too.
Items of course should be top-level tech and should introduce new possibilities or move limits within the game (train/air travel, silent lockpicking, use of electricity, etc). The mechanism can be reused for later inventions.

1. Mysterious Island with all precious resources and metals (gold, silver, diamonds and so on)
2. Teleporting gate on sea from one point of cantr world to location on opposite place of "globe".
3. Adding one new road between two chosen locations on map (even long one).
4. Yup, Kraken on sea which could crash ships(mving chars to first near land location), necessarily.

I'd like to see items given skill level so that anyone can make a bone knife, but only an expert can make a diamond encrusted sabre, for example. Or anyone can bake a potato, but it takes higher skill to bake bread or make a cake. Make the skills actually mean something. Give me a reason to keep doing something over and over.
