That sounds pretty good. It could even be used as a battle stimulant for those with low-skill-weighted weapons. (I'm not sure if that's a good or bad thing).
I also think that drunkeness should reduce all other skills as well, but have more of an effect on those which require coordination and thought.

So adrenaline makes you stronger? I don't think so. It makes you wilder, because it removes the barriers that prevent you from hurting yourself in normal situations, but does not necessarily make you stronger. It would only appear like that because it makes you behave bloody stupid.
Besides, the effect of alcohol is pretty much different from person to person. It has usually a calming effect on me and I doubt that it makes me stronger, because it prevents me from applying my strength optimally. Anyway, I'd like to hear other peoples opinions about it.

Why only the fighting skill? Let it rather reduce your overall effectiveness (like tiredness) and increase the strength value in a way that would give you a slight net bonus when using weapons with a low skill weighting.

Ummmm...doesn't both fighting and dragging involve strength as well as fighting skill, meaning in the end it'll even out anyway? :S

That would principally be the easiest way to implement it, but I would like to see the drunkenness bar put to an use. I can't give a good argument for it right now but I would prefer it, if tiredness and drunkenness would be separate things.

Caffeine also works only temporarily, leaving your even more tired after some time passes.

They don't have to even out. It would depend on how the values for the increase of strength and the decrease of overall efficiency/skill are set.
Especially when fighting, they can't even even out. At least not with all weapons. If strength would increase while drunk, skill based weapons would have a bigger disadvantage than strength based weapons. It could for example be adjusted that way that the effects of drinking would (almost) even out for someone wielding a claymore, but a person wielding a rapier or a crossbow would do considerably less damage.
I'm pretty much against alcohol having a general positive effect on fighting though.
First of all, I'm pretty sure it's not realistic. After all, soldiers usually don't get drunk before entering a battle and if they did it in past times, I guess it was more about repressing the fear and getting in the right frame of mind. Admittedly, a drunkard might be a challenging enemy, but that's rather because he behaves unpredictably, not because he's a better fighter.
Secondly, in terms of game balance, it would be nice to have something to balance the healing effect of alcoholic beverages. Most of them are very potent and there could be even more potent potions if they'd reduce your overall capabilities when consumed by making you dizzy/drunk.
