Although I should never advertise anything but Cantr, I can't resist mentioning this one:
http://www.project-entropia.com
Heavy requirements on your computer and graphics card, though ...
If I am talking about a 3D graphical Cantr, this is what I'm dreaming of
... Entropia is socially and definitely politically not as interesting as Cantr - but the map and freedom is, and the economy is quite interesting.

Aren't you forced to pay real money if you want the good stuff in that game?
Although I can understand from your perspective that you want to expand Cantr into a 3D graphical environment I kind of like that it is text based, like the old paper & pen RPGs. It gives you the oppurtunity to decide how Cantr looks like by using your imagination

True. But you don't have to pay a lot to have fun for quite a while. But you need a starting investment (e.g. 10 dollar or so).
But then, people should donate that to Cantr too

Yeah, well, but Cantr is only text-based because I was unable to do it 3D right away - would I now be bound by my own initial limitations?

I managed to get Wurm to work, I go by the name Cornelius there, I have to say it is entirely open in exactly the same ways Cantr is. Only thing is that food is as hard to gather in the game as it is in real life really and there aren't any bushes.
Really do like this game, you can walk anywhere and do anything. There's a couple of pre-placed religions but there's recently been a reset, adding the usual and playing around somewhat and so everything is blanked out. Which makes everything virgin territory.
If you want an open game like Cantr then I do suggest you try playing, it even has its own Wurmpedia site, like Wiki. Which is fairly informative and forums so no problem getting going.
The options are just enourmous though.

This is of course not a long term game and more like a puzzle, but it's kinda hard.
I finally beat it just a few minutes ago. Of course, not without help from my husband who figured it out first.
