As for my names it depends on how I'm feeling at the moment. I've named charries with simple and common names, I've used my nick, I've used my real name, I've combined my favourite fantasy names (Arrav Alavar :P Doesn't it sound good?), I've used random objects names (After someone gave my charrie a shovel he adopted that name for his surname. It sounded great, I'm sad I let him die) , I've got names from the bible and twisted them a bit, I've asked Pie for a name and twisted so that it's looks well on Portuguese xD ( Jameso Bondo--> Bruno Someja), I've used tags that I see around the town on names. One of the nicest is a nickname my charrie has because, when I was a newbie, I named him Black Hawk (--"), later, when I realised the stupid name I gave that charrie, I did another newbie act and changed the name like nothing had ever happened, changed it to John. The sheriff of that town in a good discret rp moment nicknamed me as Black John. I liked it and still like it :P

I just draw them up from air, though sometimes I'll let a few glances help me decide. For instance, there was a brand on a TV screen that I once owned, it read, "Tantus". There, a perfectly good original first, last, or middle name. It's not that hard.

I wasn't going to post since so mny people said the same: drawing the name from nowhere. However I realized I have a mmuch more complex process of name choosing: It should fit the char's language (mostly), then it should sound good, might say something about the char itself. Then I think if I aprove it or not. Sometimes I just put something, play mute newspawn and later think of a name.
