Death

I've seen several characters die without warning or notification. One person died in Lad prison for no apparent reason. He was healthy and in his twenties, and a day later he's dead without any message, no "You see **** die of hunger" or "You see **** enter and slice **** into little bits with a sabre." There were also two more cases which I won't go into detail about because of the 4 day rule, but both involved healthy young characters dropping dead with no visible cause of death. Should this happen?

Hmm, maybe disease has been implemented...the Lad prison is a pretty filthy place, what with all the bodies in there.

Nah, I don't think so.
a) The Lad prison death was a while ago.
b) The other incidents were in clean open spaces
c) I haven't heard anything about disease being implemented.
d) There should still have been a message, like "You see ***** die of disease"

No, that is because the person unsubscribed from the game. There is no message for when their characters die because of that.
