Yes, we've seen it before but for some reason it seems to be repeating a lot more lately, at least on one of my characters. I haven't used the browser back button, I've used the arrow at the bottom of the page when reading notes. Once it even kicked me out in the middle of loading down the inventory page! Two note titles were showing, and then it did it. Could it be caused by not loading the page all the way down?

Yes ,under some circumstances navigating off the page before it's fully loaded can cause this.
