Is this an error?

In seatown forest a man with only a wooden shield has managed to stay unhurt when attacked with a crossbow, a CR Bow and a sabre.
Not sure if this is an error of if hes just lucky

The attacker(s) probably have lousy fighting and physical strength skills; I used to have several characters who could only do about 7 damage to an unshielded target with a sabre.

And there is luck as well . There is a chance any character will miss , even expert fighters .

Also is your character who observed this sure that the others really did miss? the person being attacked may have been healing themselves immediately after the attack . I doubt this is an error , there are a lot of legitimate explanations
