Heart Attack Reasons?

Ok, I maybe a completely freshwoman in this game but I already know that when a player unsubs his/hers characters die instantly of a heart attack. In fact I myself did unsub only one week to the game cause I felt my characters were lousy.
I changed my mind one day later and so here I am with a few new characters trying to understand the game dynamics and mechanics.
My question is are any other reasons for a character to die of a heart attack. Like a random incident , a desease or something of the sort.
Or by not logging into the game (if that happens after how many days of not logging in?)
Or in cases of a ban?
And when a character dies of a heart attack does this always mean that all the characters of the specific player have died?

An account is automatically unsubbed if not accessed for 20 days, so that's one major cause for heart attacks. A ban is a possibility, too, but PD has the option to kill only some of a player's characters as well.

Apart from that, there are no random causes for heart attacks. It would be majorly annoying if your chars could be killed by a disease just like that.
