I guess you're missing the point here. Changing background color is not only to notice the player that the char is hungry or wounded. It's kind of a "feel the pain".
Now, you can live with a char half hounded without even care, specially if it's some char you don't care if it dies. The point is making it unpleasant so you try to stay healthy.
To make hunger and woundedness more notable...

Changing icon color might work - I liked the smiling/frowning system of the days of yore, and some form of revival of that would be very cool.
But as for the "feel the pain" justification for changing background (I'm not convinced it would have that effect, but regardless): anything that intentionally makes Cantr harder to play under any circumstances is just not nice - its likely effect would be to make players quit or use OOC methods (software, playing techniques) to work around the effect, not change their playing style. In this case, players already have enough reason to avoid getting their characters injured or hungry.

There are a long term plans which are currently in development, to move most of the formating out of the code of Cantr. One of the intentions is to move all formating to CSS , This will also allow users to develop their own skins and preferences. (The other major intended benefit is to speed up the game by removing a particluar bottleneck)
The use of the screen background color to indicate character health is really at odds with this plan and for that reason alone it is unlikely to be implemented.
