Toothbrush sounds okay, but a toothpick could easily be improvised. Just rp using a splinter of wood or a small bone.

Yes to the toothbrush! And toothpaste. Mouthwash. Tooth decay and dentures would be fun too, but perhaps that's going too far.

While I think roleplay items can be great for the game I think there needs to be some bearing on what is good for creating something to role-play off(musical instruments for example) and what is just insignificant (toothpaste, nail files, WD40 etc.).

I have meet a char that rped a toothpaste (a mixture of some soda, lemons and others), a mouthwash could also be improvised, but the biggest problem is a toothbrush.

Be great if there was a system where, if you didn't carry a toothbrush, there was some in game indication that you had bad breath, etc. (not unlike hunger)

So when we have such item to keep us clean, shoudnt we get dirty and start to smell bad? Now we can clean up, take a bath, use tools on our journey.

Since the toothbrush is a RP-item, and doesn't actually do anything, you have to RP being dirty and smelly. I had such a char once, it was kind of fun for a while.
