Somehow possible to wear multiple clothes in same slot

I noticed this morning that one of my characters was wearing two scarves. I took one off and when I put it back, it took off the other one like it's supposed to, so I can't reproduce this, but I have no idea how it could happen. Also I don't remember putting the second scarf on in the first place, although I probably just forgot.
