screwdriver disappeared

One of my charries was placing a screwdriver into repair they had in their inventory. It disappeared and never made it into repair 
. It happened around a server lag. The next thing I saw was flash back to a different charrie....it was odd. Cantr time was around 1703.
What happened to the screwdriver? It's not in inventory , no where. No one could have repaired it since they are alone.
I can pm details of the charrie, just let me know.
